How do I create and edit assessments

Assessments allow your company to capture the data necessary to create analytics, comparisons, and map your supply chain.

Assessments can be divided into sections that contain indicators (questions) and can be created from a blank template.

This article covers how to:

Create or edit an assessment

  1. Click the menu icon  in the top left corner.
  2. Click Assessments.

  1. Click the icon next to Assessments.
  2. Enter the name of the assessment.
  3. Select the target of the assessment. You can choose multiple targets.

Choosing the target of your assessment is important. It determines the assessment layout and how data can be used in analytics.

Company Requests data related to a company as a whole.
Site Requests data on specific production, mill, or manufacturing sites. Each site requires individual responses to the assessment.
Product Requests data on specific products, materials, or order number. Each product requires individual responses to the assessment.

Add or edit sections

  1. Click Add Section.
  2. Hover over the section title (Blank section) to display the edit pencil icon.
  3. Click the pencil icon.
  4. Enter the new section title.
  5. Click the check mark to save.
  6. Click, hold, and drag the  icon to move the indicator to the desired position.

Add or edit indicators (questions)

With the SupplyShift assessment editor, you can create custom indicators.

  1. Click Create New Indicator in your newly added section.

    A blank indicator is added to the section.

  2. Click Indicator blank to display the details.
  3. Click Edit Indicator.

  1. Click, hold, and drag the  icon to move the indicator to the desired position.
  2. Click Edit Indicator to change the indicator characteristics.
  3. Choose the question type.
  4. Choose other characteristics that will define the indicator.

Question types and indicator characteristics

Altitude Respondents will enter altitude coordinates On the mobile application, respondents will click a button that automatically enters their device's altitude coordinates or altitude.
GPS Respondents will enter GPS coordinates On the mobile application, respondents will click a button that automatically enters their device's GPS coordinates or altitude.
Location Respondents will select a country and a state/province from a list. This indicator can be used in the Supply Chain map for supply chain insights.
Date A calendar is displayed and suppliers select a specific date (M/D/Y).
Multi-Select The supplier can select more than one response option. Multi-Select is incredibly useful for combining questions and reducing length of the assessment. Rather than asking multiple questions, you can ask one and respondents select all responses that apply.
Multiple Choice Suppliers are shown a list of possible answers and can only select one answer from that list.
Numeric This option requires the answer to be written in numeric form.

This is best used for questions asking for count of people, percent or volumes of a product.

If you would like to limit numerical response - connect with your implementation contact.

Text Supplier can type out their response in an open text box.
Year Respondents select a year from a list.
Yes/No Respondents can only choose Yes or No options. To collect more than one piece of information we suggest using multi-select.
Add Tooltip Additional terms and explanations that aid in answering can be viewed by hovering over a ? icon. Photos, videos, and links can be embedded in the Tooltip.
Document upload options Allows suppliers to upload third party or internal documentation to their answer.

SupplyShift does not analyze or machine read comments. Your team will need to download and review comments - we suggest using comparisons for reviewing.

These cannot be required for submission - use the upload indicator type instead.

Required for submission Suppliers must answer the indicator in order to submit their assessment. Suppliers will be notified upon submission which indicators were not answered.
Comments Allow for suppliers to add comments to their response. SupplyShift does not analyze or machine read comments. Your team will need to review comments - we suggest using comparisons for reviewing.
Not applicable (N/A) Allow suppliers to determine if the indicator is not applicable to them.

Privacy status

Privacy status allow you to determine to whom your suppliers can share responses to your questions.

  1. Click on the globe or person icon to choose the privacy setting for the indicator.

    Hover over the icon to see the description of that privacy status.

Add language translations

SupplyShift supports eleven languages. Platform text is translated, however, assessment text must be translated by you.

  1. Select an indicator when in Edit mode of the assessment.
  2. Click Edit indicator.
  3. Click Language and choose the language you would like to translate into.
  4. Type your translation into the available boxes. You can translate response options and units.

    Translate all the text with an asterisk (*) preceding it into the desired language.

  5. Click Close when you are done and ready to move on to the next indicator.

    Changes to your assessment are saved automatically.

Assessments cannot be edited once they have been sent!

Creator vs respondent views

This section shows the respondent view.

  • All active indicator characteristics are displayed.
  • Indicators are affected by skip logic.
  • Matrix sections are displayed as a table.

  • Indicators do not display active attributes.
  • Indicators are not affected by skip logic.

In the assessment creator view, a matrix table is displayed as a collection of indicators.

Once you have completed all the edits to the assessment, you are ready to send to your suppliers.
